Required Documents for IDCMP

The following documents are required for enrollment and completion of the IDCMP program. Please begin obtaining these documents immediately.

Full Discovery / Arrest Report & Narrative

  • • This includes the police report and all documents related to your arrest.
  • • If you had an attorney, they can provide this to you.
  • • If you did not have an attorney, contact the arresting police department directly and ask them to send you your discovery file, including the arrest report and narrative.
  • • Also request your BAC results and any Refusal Form from the arresting police department.
  • • If you were arrested by NH State Police, use the link below to find each troop's contact information.

Certified Driving Records

We are required to collect certified driving records both from New Hampshire and from any other state that applies to you.

New Hampshire Certified Driving Record

In person

$20, payable by cash, card or check to “State of NH–DMV”. Bring a photo ID that has not been expired more than 3 years. You walk out with your driving record in hand. Pick the form up at the DMV or print it using the link below.

By mail

$20 by check or money order payable to “State of NH–DMV”. You can print the request form with the link below. A copy mailed to you does not need to be notarised; if you want the DMV to mail it directly to us, it does need to be notarised.

Online (preferred method)

$20 by card, mailed to the address on your driver licence. To order online you need your NH driver licence or non-driver ID number, and your name, address and date of birth must match what the DMV has on file.

How to complete the DMV Record Request form

  1. Step 1: check only the first box.
  2. Step 2: check the first box, “I am record holder”, then complete the two required sections — name and date of birth.
  3. Step 3: complete name, phone and address.
  4. Step 4 and Step 5: skip.
  5. Step 7: sign and date.

Certified Driving Records from Other States

  • • If you do not live in New Hampshire, we need a certified driving record from the state you reside in.
  • • We also need one from any state where you have ever had a DUI/DWI offence.
  • • All driving records must be printed or updated within the last 60 days.

Surrounding-state DMV/RMV guidance

For clients with a DUI in Maine, visit a Maine BMV to obtain a 10-year certified driving record or contact the Maine BMV at 207-624-9000 to request one by mail.

New Hampshire Criminal Record

  • • Required by law. Cost is $25, paid by debit or credit card online.
  • • Order online using the link below, then choose “Request Online NH Criminal Conviction Check”.
  • • When you order it online it is emailed to the IDCMP within 1–2 business days — upload it to the portal once you receive it.

Court Order / Case Summary

  • • Submit a copy of your court order from the day of your conviction, or call 1-855-212-1234 and ask the court to email you your Case Summary.
  • • Case Summaries are usually sent the same day and can be uploaded to the portal.
  • • If you had an attorney they have a copy; ask them to email you the packet to forward to us.

Qualified Victim Impact Panel

  • • Effective 1 January 2026 this is New Hampshire law: every DWI client must complete a court-approved Victim Impact Panel before booking their 20-hour class.
  • • It is completed online, takes about 3.5 hours, is available 24/7, and is paid for by the client to the provider.
  • • It focuses on the real-life impact of alcohol and drug-related driving on individuals and families.
  • • The fee is paid directly to the provider you choose, and providers charge different fees.
  • • Proof of completion is required before your class registration can be finalised. You must submit it to us AND to the DMV at reinstatement — the DMV verifies completion with the provider.

Proof of Hardship

You will need proof if you receive TANF, SSDI, SSI, APTD or VA Disability benefits and you are a NH resident. Once you provide proof of being on one of these programs, you will qualify for hardship discounts. Proof must be submitted prior to intake/screening along with proof of residency.
